Pour le papier jaune ca peut se faire dans le commissariat de votre quartier mais faites vous accompagner par qq'un qui parle et ecrit chinois et prenez des photos (de toutes facons pour toutes les formalites prenez des photos!) et passeport. Dans ces conditions c'est tres facile. Sinon il existe une sorte de commissariat central pour cette formalite.
Si je me rappelle bien la visite medical se fait avant.
et ensuite la demande visa dans un batiment du centre ville. Je recheche l'adresse et je re-post.

5 working days are requested to issue the residence card usually.

Foreigners holding D, Z, x, J-1 shall within 30 days of entry into China obtain aliens residence cards or aliens temporary residence cards from the city of country public security bureau at the place of their residence.

Recently, residence cards can be granted for a period of 3 or 5 years instead of one but you still have to apply for your visa every year.

As an employee in Shanghai, you should submit:

* Application letter form the employer
* A business license transcript of a legal entity in China or the registration certificate of a representative office of a foreign enterprise
* Qualification certificate issued by the Shanghai municipality foreign trade and economy committee or other relative authorities
* Expert's certificate or employment registration for foreigners issued by the Shanghai Labor Bureau
* Health certificate, temporary residence certificate issued by a hotel or the police of the applicant's residential district
* Two and a half inches length photo and passport

As a dependant of a foreign employee in Shanghai:

* Spouse should submit in addition to the above document
* Upon issue of the resident card of the employee
* Marriage certificate
* Health certificate (applicants under 16 years old are exempted)
* Tempory residence certificate
* Two and a half photos
* Passport
* The foreign employee's residence card
